We value your time and are committed to providing a personalized, attentive experience for every patient. To ensure appointment availability and allow adequate time for each visit, the following cancellation and deposit policy applies.
1. Appointment Deposits
A deposit is required at the time of booking to secure your appointment.
· $50 Deposit: Primary care, wellness, weight management, longevity, IV therapy, and consultation appointments.
· $100 Deposit: Aesthetic treatments, including neuromodulators, dermal fillers, PRP/PRF, and microneedling.
All deposits are applied toward the total cost of your scheduled appointment.
2. Cancellations & Rescheduling
We kindly request at least 48 hours’ notice if you need to cancel or reschedule your appointment.
• 48 hours or more: Your deposit may be refunded or transferred to a rescheduled appointment.
• Less than 48 hours: Your deposit will be forfeited as a late cancellation fee, unless an exception is approved.
3. Missed Appointments & Late Arrivals
Patients who do not attend their scheduled appointment without prior notice will forfeit their deposit. If you arrive late, your appointment may need to be shortened or rescheduled to avoid affecting other scheduled patients. If rescheduling is necessary due to late arrival, the deposit may be forfeited.
4. Exceptions & Practice Cancellations
We understand that unexpected circumstances arise. Exceptions may be considered for illness, emergencies, or other unforeseen circumstances at the discretion of ELARIS North Shore. If ELARIS North Shore must cancel your appointment, your deposit will be refunded or transferred to a new appointment at your preference. .
5. Cookies & Analytics
Deposits are credited toward the scheduled service and are not additional charges. No separate cancellation or no-show fee will be assessed in addition to forfeiture of the deposit for the same appointment. Membership cancellation and payment terms are governed separately by the applicable Membership Agreement.
